Simon Peter Moore is the founder of Code-4Forums, a professional learning platform focused on strengthening incident readiness in schools and community organizations. With 15 years of EMS operations service and 20 years in education, combined with disaster preparedness certifications as emergency management specialist, American Heart Association instructor he brings a practical, cross-sector perspective to preparedness—helping teams build clarity and confidence for the critical minutes before arrival of first responders.
Through Code-4Forums and the Incident Management Summit, Simon convenes educators, administrators, and response partners to translate planning into action through training, tools, and real-world application.
Simon holds a BA in English, MS degree in Health Services Administration, Masters in Education with administrative credential, and is a doctoral candidate in leadership with a public health education focus at the American College of Education. His dissertation case study, “Staff Response to Emergency Incidents in California K-12 Schools” will illuminate the topic, generating discussion which brings systems closer to solutions.
Dr. Monica Maybank is a strategic advisor, leadership consultant, and bridge builder dedicated to helping organizations navigate change and achieve long-term success.
With a background in public policy and executive leadership development, she has built a reputation for fostering collaboration across industries and ideologies.
After nearly a decade as a federal lobbyist, Dr. Maybank founded The Almond Group. Monica and her team work with executives, leadership teams, and government affairs organizations nationwide, guiding them through organizational strategy, leadership development, and long-term vision execution.
Monica holds an Executive Certificate in Strategic Management from Georgetown University. She earned a bachelor’s degree in journalism from Howard University and a doctoral degree in education policy, evaluation, and reform from Claremont Graduate University.
Monica is a proud wife to a U.S. Army combat veteran and a mother of four daughters.
Jason Tate is a 20+ year secondary education veteran with a B.S. in Biology and a Functional Medicine Health Coaching credential, supported by extensive training in nutrition, education, mind-body medicine, and wellness coaching.
For the past 17 years, he has presented to audiences of all ages on motivation, movement, nutrition, and mindfulness. A former biology teacher, varsity volleyball coach, and science department chair, Jason developed four UC/CSU-approved science courses and created the nation’s first functional medicine pre-med academy built on three pillars of wellness: movement, nutrition, and mental health.
He hosts the “Tate Talks” podcast, has appeared in multiple media outlets, and delivered a TEDx Talk in October 2021. Jason currently serves as Wellness Specialist at C.V. High School, where he recently secured funding to build an 1,800-square-foot Zen Garden and helped support recognition as one of America’s Healthiest Schools.
Kelly Smith, RN, CEN, TCRN, CPST is a Registered Nurse with more than 17 years of clinical and public health experience and currently serves as the Injury Prevention Coordinator at Riverside University Health System Medical Center. With a strong foundation in trauma nursing, emergency care, and community health, she is dedicated to reducing preventable injuries through evidence-based education, outreach, and collaboration.
In her current role, Kelly develops and leads injury prevention programs focused on reducing falls, motor vehicle crashes, violence, and other unintentional injuries. Her work extends beyond the hospital, partnering with emergency services, schools, and community organizations to identify risk factors, deliver targeted interventions, and improve safety outcomes across diverse populations.
Kelly is a passionate educator and Stop the Bleed instructor, having trained more than 6,000 community members, including healthcare providers, first responders, students, and international audiences. She also serves as Injury Prevention Chair for the Emergency Nurses Association (ENA), contributing to national initiatives, resources, and advocacy efforts. Through leadership, service, and a global public health perspective, Kelly is committed to building safer, healthier communities locally and abroad.
